Eyelid surgery (blepharoplasty)

With age, the skin of the eyelids loses elasticity and begins to sag, and the fatty tissue that supports the eye can shift, creating a puffy appearance. The result is drooping upper eyelids that can narrow the visual field, and baggy lower eyelids that give the face a tired, older look.

When is it needed?

Blepharoplasty is a surgical procedure that removes excess skin, muscle, and fatty tissue from the eyelids, restoring their natural, rested appearance and - when needed - also improving visual function.

Blepharoplasty can be functional or aesthetic - and often both at the same time.

Upper eyelids are operated on when excess skin hangs down so much that it obstructs the visual field or interferes with daily activities. This is a medical indication, not merely a cosmetic one.

Lower eyelids are operated on when there are pronounced undereye bags, puffiness, or excess skin that gives a tired appearance, regardless of how rested you are.

Good candidates for the procedure are people in good general health, without serious eye diseases, with a well-planned intervention and a consultation with the surgeon about the expected outcome.

Before the operation, an eye examination is essential to rule out dry eye and other contraindications, and to assess whether the indication is functional or aesthetic.

How is it performed?

The procedure is generally performed under local anaesthesia, depending on whether the upper eyelids, lower eyelids, or both pairs are being operated. Incisions follow the natural folds of the eyelid, so scars are practically invisible after healing.

On the upper eyelid, a carefully measured amount of skin and, if needed, fatty tissue is removed. On the lower eyelid, the approach can be external (through the skin below the lashes) or internal through the conjunctival mucosa (transconjunctival approach) when only fatty bags are removed without excess skin. As it is performed as day-case surgery, the patient goes home the same day.

What to expect?

In the first few days, swelling and bruising around the eyes are normal and gradually subside over one to two weeks. Cooling, rest, and temporary avoidance of physical exertion and make-up are recommended. Sutures are removed after 5–7 days, and the final result is visible after several weeks when swelling has fully resolved. Scars are discreet and located in the natural folds of the eyelid.

How long do the effects of blepharoplasty last?

The effects of eyelid surgery are long-lasting. The excess tissue that has been removed does not return, although the skin continues to age naturally. With good care and sun protection, results are maintained for longer.

Is the procedure painful?

The operation is performed under local anaesthesia, so you do not feel pain during the procedure. After the procedure, mild tension and discomfort are possible, which are easily managed with standard analgesics.

Can both upper and lower eyelids be operated on in the same session?

Yes, upper and lower eyelids are most often operated on together - this shortens overall recovery time and reduces the number of procedures. The decision depends on the indication and the surgeon's assessment at the consultation.
