Laser iridotomy

Laser iridotomy is a short, painless procedure in which a small opening is created in the iris with a laser. Milmedic Clinic has the latest generation NdYAG laser for anterior segment procedures.

When is it needed?

The eye constantly produces aqueous humour that circulates within the eye and drains through natural channels. In some people, the anatomical structure of the eye is such that the angle between the iris and cornea is narrowed, making drainage difficult or completely blocked. The result is a rise in intraocular pressure that can damage the optic nerve and lead to narrow-angle glaucoma.

Laser iridotomy is indicated in two situations:

  • Therapeutic - when acute or chronic angle closure with raised pressure has already occurred

  • Preventive - in people with an anatomical predisposition (narrow angle, shallow anterior chamber) to prevent an acute glaucoma attack. For this reason, the procedure is often performed on both eyes.

How is it performed?

It is performed on an outpatient basis. The patient sits in front of the laser while the doctor places a special contact lens on the eye. The NdYAG laser then precisely creates a small opening on the periphery of the iris, establishing direct communication between the anterior and posterior chambers of the eye. Aqueous humour can now freely bypass the blockage and pressure normalises.

The procedure is performed under local anaesthetic drops, takes only a few minutes, and is completely painless.

What to expect?

Immediately after the procedure, the doctor checks intraocular pressure. You may temporarily notice a mild glare or line at the edge of your visual field - this is normal and passes. A follow-up appointment in the following days is recommended to monitor pressure.

Frequently asked questions

Can the opening made by the laser close?

In rare cases the opening may partially reduce in size, which is why the condition is monitored at follow-up appointments. If needed, the procedure can safely be repeated.

Does iridotomy treat glaucoma?

Laser iridotomy is not a treatment for glaucoma in the general sense - it is a specific solution for narrow-angle glaucoma, where it prevents blockage of aqueous drainage. For open-angle glaucoma, this procedure is not indicated and other treatment methods are used.

Will the opening in the iris be visible?

The opening is made on the periphery of the iris and is practically invisible to the naked eye - neither to you nor to others. It does not affect the appearance of the eye.
