Refractive Lens Exchange (RLE)
Refractive Lens Exchange (RLE) is a refractive procedure in which the natural lens of the eye is replaced by an artificial implant of precisely calculated refractive power. Unlike laser methods that alter the curvature of the cornea, RLE addresses the source of the refractive error from within.

When is it needed?
RLE is a particularly suitable option for people over 40 for whom laser methods are not the optimal solution, as well as for those with high refractive errors or thin corneas.
The procedure is used for myopia, hypermetropia, astigmatism, and presbyopia - the age-related loss of lens flexibility.
Since the natural lens is removed, patients who have had RLE cannot develop a cataract in the future.
How is it performed?
The technique is very similar to cataract surgery - one of the safest and most commonly performed operations in the world. The natural lens is removed by phacoemulsification, and an artificial, foldable implant is inserted through the same incision and unfolds in place of the natural lens.
Premium lenses are available - multifocal, aspheric, and accommodating - chosen according to each patient's needs and lifestyle. Your ophthalmologist will recommend the most suitable lens type after a detailed examination.
Which lenses do we use?
At Milmedic Clinic, we use top-quality lenses with no expiry date, following the most advanced protocols that meet international standards. We most commonly implant premium lenses - the advanced generation of intraocular lenses that provide sharp vision at multiple distances. Depending on the patient's needs, we choose between EDOF lenses (extended depth of focus) and multifocal lenses, all on the Clareon platform - the most advanced platform in modern ophthalmic surgery.
What to expect?
Recovery is very fast and outcomes are highly predictable. Most patients achieve excellent quality of vision without glasses for distance, and - depending on the type of lens implanted - for near as well.
As with cataract surgery, the procedure is performed in an operating room under local anaesthesia.
RLE at Milmedic Clinic is performed by an experienced team: Prof. Dr Miroslav Vukosavljević, Dr Nenad Petrović, Dr David Simonovski, Prof. Dr Mirko Resan.
Frequently asked questions
Can RLE be performed on both eyes on the same day?
Under the standard protocol, the procedure is performed on one eye, with the second eye operated on in a separate visit - usually after one week. This allows for a safer recovery and confirmation of results before approaching the second eye.
Can astigmatism be corrected with lens implantation?
Yes - toric lenses are used to correct astigmatism. They are specially designed to neutralise irregular corneal curvature. Your ophthalmologist will determine based on pre-operative measurements whether a toric lens is the right solution for you.
What is the difference between RLE and cataract surgery?
The technique is identical - in both cases, the natural lens is removed and replaced with an artificial implant. The difference is in the indication: in cataract surgery the lens is cloudy and must be removed, whereas in RLE a healthy lens is removed to correct high refractive error or presbyopia.
Clear vision without glasses - for good.
