Selective laser trabeculoplasty (SLT)

When is it needed?

In open-angle glaucoma, raised intraocular pressure occurs because the natural filter through which aqueous humour leaves the eye is not functioning sufficiently well. SLT is used when eye drop treatment does not achieve the desired pressure, when the patient cannot use drops regularly, or as a supplement to medication therapy.

How is it performed?

It is performed on an outpatient basis. The patient sits in front of the laser while the doctor places a special contact lens on the eye to clearly view the drainage angle. The low-energy laser beam (0.3–2 mJ) acts in very short pulse intervals of 3 nanoseconds, practically eliminating any thermal effect on surrounding tissues.

The target of the laser beam is exclusively the melanin-rich cells of the trabecular meshwork. Surrounding structures of the eye are completely unaffected - this is precisely the essence of the selectivity of this method. SLT stimulates the eye's natural biological response, which improves drainage of aqueous humour and thereby lowers pressure.

During the procedure, at certain energy levels, a "champagne effect" may occur - small bubbles on the trabecular meshwork - which is a sign of effective laser action.

What to expect?

The effect is usually noticed within a few weeks to months. SLT can lower intraocular pressure by 6–8 mmHg, and in most patients the effect lasts for several years. One of the key advantages of SLT is that, unlike older laser methods, it does not leave permanent scarring, so the treatment can safely be repeated if needed.

After the procedure, a mild transient rise in pressure or moderate eye irritation is possible, so a follow-up appointment is recommended.

Does SLT replace eye drops for glaucoma?

In some patients SLT can fully replace drops, while in others it serves as a supplement to existing therapy. The goal is always to achieve the target intraocular pressure - the doctor assesses whether this is possible with SLT alone or in combination with medication.

Is SLT painful?

The procedure is painless. During the procedure you may feel mild pressure, and after the procedure brief mild eye irritation is possible.

Can I have glaucoma surgery in the future after SLT?

Yes, SLT leaves no permanent changes to the eye tissue and does not exclude any future treatment. If SLT does not achieve sufficient pressure reduction, or the effect diminishes over time, all surgical options for glaucoma treatment remain available.
